Please find below, instructions to using online counseling services. Please read carefully. Instructions must be followed precisely. This will ensure maximum potential for confidentiality.

Step 2: Download and read the Online Counseling Declaration of Policies and Procedures/Informed Consent form.

Step 3: Download and read the Emergency Contact form.

Step 4: Sign both forms and fax the Emergency Contact form and the signature page from the Informed Consent form (only the signature page is required) to 423-000-0000. I must have both of these forms filled out properly and signed before we can proceed with online counseling. If you have any question about the forms you can contact me by Clicking Here.

Step 5: Once steps 1-4 are complete, proceed to the options below for online counseling. I currently offer email and instant message/chat formats.

Option 1: Quick Question Email Counseling This option will allow you to write an explaination of what is wrong and I will provide a one time response. Please be as detailed as possible and I will give you as much information in my answer as possible. I will respond within 48 hours. The cost for this option is $35.00 USD. Please click the payment button below to proceed with this option. You will be transferred to a secure area of this site. You will fillout the form and be given a unique, secure ticket number with instructions for how to retrieve the response to your email.

Option 2: One week worth of Email Counseling This option will allow you to correspond with me as much as needed for one week. I will respond to each email within 48 hours. The cost for this option is $75.00 USD per week. Please click the payment button below to proceed with this option. You will be transferred to a secure area of this site. You will fillout the form and be given a unique, secure ticket number with instructions for how to retrieve the response to your email.

Option 3: Instant Message/Chat Session Selecting this option will give you access to the secure instant message/chat software on this site. I will provide you with a 45 minute session to talk about your issues one-on-one. The cost for this option is $95 USD per session. You can have as many sessions as you need. You will need to fill out the form with your contact information so that we can schedule a time to meet online for our Instant Message/Chat session.
